Evaluate your current physical condition and how much your knee pain is affecting your life. Print out this short assessment, fill in the answers, then bring the assessment to your orthopaedic surgeon to discuss your knee pain treatment options.

  • Do you have knee pain that limits your ability to play sports, do your job, take care of household duties, or attend social/family events?

  • Does the pain get worse when you are active?

  • Is your injured knee swollen, or do you have other symptoms, such as clicking, locking, or catching?

  • Do you find yourself having to take pain medication due to knee pain?

  • Have you ever seen a doctor about your knee pain?

  • Have you had one or more surgeries to attempt to repair the injury or improve your symptoms?

  • Are you dissatisfied with the results of your previous surgery(s) for this injury?

  • On a scale of 1–5, how limited have you been in performing activities that you once enjoyed? (1 being not limited, 5 being very limited)

  • Are you interested in learning about a treatment option that may help you to resume your prior level of daily activity?

  • Are you willing to participate in a rehabilitation program following surgery?
